Someone bought the Peak product in the first frame and thought it was powdered milk, alas it was a new milky pap product by the company.

This made me think about product designs in a Human factors perspective
we have to create systems that prevent errors and not insist on people being more careful, for to err is human.

In the second frame, you can see how two different medications: Metformin and Metronidazole are in very similar packs.

This is an error waiting to happen.

You can imagine the impact of giving metformin for treating of infection when you actually meant to dispense Metronidazole.

We need to make our systems safer by making it difficult for healthcare workers to make errors.

let's make it easy for them to do the excellent work, they planned to do.
